New Driver License
Documentation Requirements

Print the New Documentation Requirements

Starting January 1, 2010, when you go to a driver license office, you must bring original documents that prove your identity, Social Security number, and residential address.

  1. Identity: Present the following original identification documents as proof of your identity:

  2. Certified U.S. birth certificate (including territories and District of Columbia), valid U.S. Passport, or Original Certificate of Naturalization (no copies or hospital certificates will be accepted).
  3. AND
  4. A Florida or out-of-state driver license or identification card, or an acceptable secondary form of identification.

    Acceptable secondary forms of identification:
  5. Certification of naturalization, Form N-550 or Form N-570
  6. Consular report of birth abroad
  7. Certificate of Citizenship, Form N-560 or Form N-561
  8. Original name change documents for all (marriage, divorce, adoption, legal name change, etc.) must be presented at the time of application (may require more than one).
  9. Social Security Number (BRING ONE) Bring one of the following original documents** as proof of your Social Security number, if issued:

  10. Social Security card
  11. W-2 form
  12. Pay stub
  13. SSA-1099
  14. Non-SSA 1099 (Click here for acceptable variants for Form 1099)

  15. **Note: The document presented must include the customer's full name and Social Security number to be accepted.
  16. Residential Address (BRING TWO) - The address on a driver license or ID card can no longer be a post office box. Bring two proofs of residential address.

  17. Deed, mortgage, monthly mortgage statement, mortgage payment booklet, or rental agreement.
  18. Florida voter registration card
  19. Florida vehicle registration or title
  20. Florida vessel registration or title (if living on a vessel/ houseboat)
  21. W-2 Form or 1099 form
  22. Utility bill, hookup, or work order not more than 2 months old
  23. Current automobile or homeowner's insurance policy or bill
  24. Mail from financial institutions, not more than 2 months old
  25. Selective Service card
  26. Medical or health card with address listed
  27. Un-expired professional license issued by a government agency in the U.S.
  28. A letter from a shelter or half-way house verifying that applicant lives at the address
  29. Educational transcripts for current school year
  30. Transients - sexual offender/predator/career offender: FDLE Registration form completed by local sheriff's office.
  31. Statements:
  32. Customers under 18 - a statement from a parent, stepparent, or legal guardian of the minor is required.

    The parent, guardian, or stepparent must reside at the same residential address as the minor. The parent, guardian, or stepparent who accompanies the minor will give a verbal statement that the minor resides at the same address and present two (2) proofs of residential address. The parent, guardian, or stepparent who cannot accompany the minor must complete a statement through a Certification of Address form with two proofs of residential address.
  33. Customers 18 years or older - customers who reside with someone must provide a statement through a Certification of Address form from that person, along with two (2) proofs of residential address in that person's name.
